JACC (JERSEY ASSISTANCE FOR COMMUNITY CAREGIVING)
- Broad array of in-home services and supports that enable an individual (age 60 and over) at risk of placement in a nursing facility to remain in home. Income-eligible program ($1732/mo, assets under $40,000). Not for those on Medicaid or Medicaid waiver. Copay.

CAP (CAREGIVER ASSISTANCE PROGRAM)
- Program to assist eligible individuals (age 65 and over) remain in or return to the community rather than be cared for in a nursing facility by offering community-based services to supplement the assistance of that person's own caregiver(s). Monthly income limit $1732, resources under $2000 (2005). Services include case management, homemaker, respite, emergency response system, social adult day care, transportation, home meals, and more.
