PSRC provides help determining whether you are eligible for state or federal benefits, and provides assistance with applications.
A care coordinator will help an individual or family link to services that are needed to support aging in place in one's home. A professional assessment of strengths, needs and risks will be done, with ongoing contact with a coordinator, your "go-to" person.
