Care Coordination or "case management" services are available to low and moderate income residents of Princeton. Individuals and their family caregivers are linked to a PSRC social worker who becomes their "go-to" person. The care coordinator does an assessment of needs and resources, and makes recommendations for how these needs can be addressed. The care coordinator will help people learn about the array of local services and link them to area resources. The care coordinator maintains contact over time as needs change, and can meet with the individual, couple and family either in the home or office. The goal of this program is to help people remain independent in their own homes with dignity and self-determination as long as possible, and to assist them in navigating the transitions of the second half of life.
This service does not currently offer 24/7 coverage. Those wishing greater coverage or additional services are referred to area Geriatric Care Managers.
