Partners In Caring
Partners In Caring is a collaboration of six social service organizations helping low and moderate income seniors and people with disabilities to lead safe and independent lives. The project, funded largely by the United Way of Greater Mercer County, provides these services:
- Caregiver Support for family caregivers-counseling, support groups, respite.
- Care coordination--helping people find and connect with the services they need, both within and outside the collaborative. Care coordinators do an assessment of each persons needs, risks and available resources and create a care plan. Care coordinators also check in periodically with phone calls or home visits.
- Counseling and consultations-helping people with the transitions of the second half of life, helping families make plans, and facilitating communication.
- Education-on a variety of topics related to healthy aging
- Emergency Preparedness-tools and information
- Home Delivered Meals and Kosher home delivered meals five days a week, one or two meals per day.
- Information and Referral-helping you navigate the maze of services, benefits and resources.
- In Home Safety Assessments
- Home Modifications
- Nursing assessments and follow-up
- Transportation-door to door transportation for medical appointments and other activities.
- Volunteer support- volunteer visitors do various tasks such as daily calls, errands and social visits.
There are modest fees for some of these services.
PSRC provides care coordination, caregiver support, counseling and consultations, education, information and referral, safety assessments and volunteers for older adults and their caregivers in the Princeton area. The other partners include Jewish Family & Children's Services, Enable, American Red Cross, Ride Provide and Reassurance Contact. Services are available to residents of Hopewell, Princeton, East and West Windsor, Upper Lawrence, and some of the neighboring communities.
